>[!meta] >[Drug_Class:: antiarrhythmic] >[Indications:: [[Ventricular Tachycardia]]] # Considerations ## contraindications - **don’t use amio** or any other AV blocker in *WPW* ==this is controversial; some are using in WPW== but flecainide and procainamide preferred - pregnancy and lactation ## drug interactions - warfarin → reduce dose by 25 % - increased levels of - digoxin - quinidine - procainamide - phenytoin - theophylline # Dose > only compatible with 5% dextrose - oral loading dose - 800 - 2000 mg TDS for 24-48hours for rapid loading - 200 mg TDS for 2 weeks, then 200 mg/day for slow loading - IV loading - 2 - 5 mg/kg IV (150-300mg) - over 2-15 min in emergency, otherwise over 20-60 min - maintenance 15mg/kg in 500mL 5% dextrose over 24 hours (max 900)
