# Simple febrile seizure
**Fever and ALL of the following:**
- generalised tonic-clonic
- duration <15 min
- complete recovery within 1 hour
- do not recur within the same febrile illness
> - no investigations are indicated in simple febrile seizure where focus of infxn identified and child is well
> - ix for source of fever should be guided by nature of presenation
> - no role for EEG in simple febrile seizure and limited in complex febrile seizure
> - consider CNS imaging in prolonged or focal febrile seizure
*look for following risk factors that make simple febrile seizure unlikely*
- <6 months of age (consider [CNS infection](https://www.rch.org.au/clinicalguide/guideline_index/Meningitis_encephalitis/ "CNS infection"))
- \>6 years of age
- any features of a complex febrile seizure
- signs of [CNS infection](https://www.rch.org.au/clinicalguide/guideline_index/Meningitis_encephalitis/ "CNS infection")
- previous afebrile seizures
- progressive neurological conditions
- developmental delay or regression
## investigations
- No investigation
- well child with simple febrile convulsion
- clear source of fever on ix that doesn't require investigation
- immunised
- BSL
- all patients essentially
- urine specimen
- girls <2 years
- try and get clean catch
- urine supra-pubic aspiration
- in kids with history of UTI
- blood culture
- unwell children with concern of bacterial illness
- CXR
- only if concern of pneumonia (crackles, tachyponea, hypoxia)
- LP
- unwell kids with concern of [[Meningitis]]
- FBC
- unwell children
# Complex febrile seizure
**Fever and ANY of the following**
- focal features at onset or during seizure
- duration >15 min
- incomplete recovery within 1 hour
- recurrence within the same febrile illness
