- nonspecific inflammation and hypertrophy of synovium of hip joint
- **most common cause of acute hip pain** age 3-10
- recent URTI in 50% of cases
**history**
- hip pain, unilateral or gorin most common
- some report medial thigh or knee pain
- crying at night
- limp
- often history of trauma may be predisposing factor or cause
**exam**
- usually no fever
- mild restriction in motion, esp to ABduction and internal rotation
- may be painful on passive movement or tender to palpation
**Ix**
- WCC may be elevated
- ESR may be slightly eelvated
- normal XR
- USS usually identifies effusion
- 1 in 4 have bilateral effusions
- hip aspiration to exclude other causes
**Management**
- admit and observe if [[bone and joint infection (paeds)|septic arthritis]] not excluded
- analgesia, anti-inflammatories
- heat and massage
- review in 12-24 hours
- usualy improve in 24-48 hours with rest
- consider alternative dx if sx persist for 7-10 days
