This catheter does not reach the medial end of the clavicle _(dotted white arrow)_ before descending, and its tip _(black circle)_ is oriented over the spine directed away from the shadow of the superior vena cava _(solid white arrow)_. Suspect arterial placement if the return flow is pulsatile.
